Please fill out this form with a parent.

In the Signature field, the parent must sign, not the minor. 

In the fields requesting documentation, you must include the following:

• Birth certificate

• Minor's ID (State ID, School ID)

• Parent/Guardian's ID

If you don't have the birth certificate, you may come to the shop & pick up our form to be notarized BEFORE your scheduled appointment.
